The Centre is Closed, But The Community Stays Alive
The health centre is shut down, and the doctors simply feel empty, with nothing to do. However, the islanders don’t want to see them go stale, so they make up silly excuses to keep the doctors busy.
Even though this is a small detail, it captures the essence of the storyline: the people. Over the past twelve episodes, these doctors stopped being outsiders. They became family. We initially saw how the islanders despised the doctors and medications. But now, they admire and respect them. This is how real change occurs.

Ju-Cheon and Jeong-Seon’s Heartbreaking Loss
This is probably one of the emotional parts of the series. Jeong-seon experiences severe pain, and she denies the emergency boat, worrying that the people will know about her pregnancy. However, she suffers an ectopic pregnancy and loses the baby.
It’s a devastating moment, made even more painful by Jeong-Seon’s confession that part of her feels relieved. She calls herself pathetic, but Ju-cheon, however, accepts her viewpoint. Instead of arguing, he simply admits that he was terrified because everything happened so quickly. The series deserves credit for allowing both characters to mourn differently without making either perspective feel wrong. Peak maturity.

Ji-Ui Never Stops Being a Doctor
Even after the centre is shut down, Ji-Ui doesn’t get rid of his principles. When Byeong-Cheol, the man behind the centre’s shutdown, is seriously injured in an accident, the doctors immediately treat him despite knowing they’re breaking the law.
Once again, Doctor on the Edge asks a simple question: what does it truly mean to be a doctor? For Ji-Ui, the answer has never changed. Patients deserve treatment regardless of whether they’ve been kind or cruel.

Corruption Ends
The governor was the sole person who was abusing his power from the start. So seeing him finally fall down felt satisfying. After Byeong-cheol gives evidence on the governor’s corruption, Ji-ui publicly exposes the bad guy. The sequence is surprisingly humorous as the governor tries to flee reporters, only to slip and injure himself. Now he knows how serious health centres are because who is going to treat him now?
It’s one of the finale’s most satisfying scenes, not because Ji-Ui refuses treatment out of vain, but because he forces the governor to realise his sins.

A Goodbye That Feels Earned
Thankfully, the centre is reopened, new funding arrives, outdated equipment is replaced, and news spreads that the governor has been arrested. Yay! But the sad news also arrives. The doctors’ rural placement ends as they complete their mandatory one year.

Chi-Yeon leaves the island carrying armfuls of gifts from the villagers. He feels extremely sad. Jeong-Seon chooses love and follows Ju-cheon as she doesn’t want the distance to end their relationship.
Then comes Ji-ui and Ha-ri, the main leads. Encouraged by Ms. Hwang and inspired by her late grandmother’s unwavering belief in her, Ha-Ri decides to pursue her own dream of working at the hospital instead of remaining on the island forever. Their final kiss isn’t dramatic or flashy. It’s all about two people who healed each other and decided to stay beside each other forever.
One Final Assignment

The most iconic end is when Ji-ui reveals that his next posting is in a prison. Meaning, his mission has become bigger. The final group photograph beautifully closes the series. This was never just a medical drama or a romance. It was about a community finding hope together, especially how islanders find hope and courage in modern medicine and young doctors.
Final Thoughts
Doctor on the Edge delivers a heartfelt finale. Though it isn’t similar to other hospital dramas, this one had its part in melodrama without exaggeration. Every major character received a meaningful closure, and the romance stories ended on beautiful, happy endings.
